• A certificate noting your dedication will be staked next to a flag at the Miles of Flags display in Madison Avenue Central Park July 3-6.
• Your tribute, based on the information you submit, will be listed on an online dedication page on this website.
• Your donation will support local programs and community efforts of the Rotary Club of Derby.

To sponsor or dedicate a flag, either print the registration form and mail it or drop it off with your payment (cash or check made payable to Rotary Club of Derby) to Derby Public Library 1600 E Walnut Grove, Derby, KS 67037.
